- Cancel with 30 days’ written notice
One-time $150 enrollment fee, with discounts for families, employers and early enrollment. Not currently enrolling ages 65+ due to Medicare restrictions. Pricing is subject to change.

A simple monthly membership gives you direct access to your provider and prompt, unhurried primary care visits by phone, video, or in person. We remove insurance billing from primary care so the focus stays on you.
